Service Coordination
The AHBHC Program is not intended to be a “stand-alone”
program; rather it is intended to join together or enhance other initiatives.
One of the tasks of the AHBHC Worker is to get to know and to have
a list of the programs and services available in the community for
families with children from prenatal to age 6.
The reason for creating this list is to ensure that families are
aware of the services available and how to use those services.
Some examples of programs / services available in the Aboriginal
community are: CAPC, CPNP, Aboriginal Headstart, Nurses, Elders,
Social Service Workers, Midwives, Early Years, CHR’s, Community
Wellness Workers.
The AHBHC Worker may also be invited by their clients to attend
a consultation with nurse, elders, social service workers, elders,
or midwives etc.
The AHBHC Worker may also be required to attend case management
or case conferences with their co-workers.

What are Client-Base Services?
Client-based services are one-on-one activity involving the main caregivers
of a child or children. A client is defined as a person who receives
services from an AHWS-funded program, and for whom case-records or
services notes are kept. Each client must have a unique client identifier
(i.e. client ID number.)
