AHBHC Worker connects or links the family to programs and services to support baby/ child healthy development.

To do this, the AHBHC Worker visits or meets with Aboriginal and non-Aboriginal organizations / services in their community to gather information.

The purpose of the AHBHC Worker getting to know people at these agencies is to:

  • Increase the client / family knowledge of and access to services.
  • Design a way to ensure that community resources can follow-up with maternal/new born assessment within 48 hours or 72 hours of return to home.
  • Develop protocols with birthing agencies to notify community resources about new births and discharge.
